Cervical stenosis and myelopathy

Cervical myelopathy occurs when the spinal cord is compressed in the neck. It can progress gradually and affect the hands, balance, strength and walking.

Clinical information

Understanding the diagnosis

Stenosis can result from degenerative changes in the discs, joints and ligaments. Pain intensity does not always reflect the degree of spinal-cord involvement.

The aim of assessment is to identify clinical signs of myelopathy, locate the compression and examine spinal alignment.

Symptoms and context

Signs of possible spinal-cord involvement

  • Loss of hand dexterity
  • Balance problems or falls
  • Progressive difficulty walking
  • Weakness or stiffness in the arms or legs
  • Urinary changes associated with other neurological signs

Alternatives

Options that may form part of the plan

01

Clinical and imaging follow-up in selected situations

02

Anterior cervical decompression

03

Posterior decompression

04

Stabilisation when instability, deformity or reconstruction makes it necessary
